Clinical Supervisor (Hospital)

"A clinical supervisor is a trainer who is selected and appropriately trained to be responsible for overseeing a specified trainee’s clinical work and providing constructive feedback during a training placement. Some training schemes appoint an educational supervisor for each placement. The roles of clinical and educational supervisor may then be merged."  General Medical Council (GMC) February 2011

Accountability:

Reporting: Educational Supervisor

Recommended Session Allocation:

Supporting Programmed Activities:  0.25 Programmed Activities per trainee per week (from existing Supporting Programme Activity allocation, unless explicitly stated).

Contracted:

  • Trainee assessments and constructive feedback
  • Supervisor report
  • Communication with Educational Supervisor
  • Participation in annual General Medical Council (GMC) Trainer survey 

Comment:

Requirement to be familiar with trainee's curriculum and portfolio. Clinical Governance role, to ensure that clinical supervision, appropriate to the competences and experience of the individual trainee occurs at all times to ensure patient safety.

Recommended Session Allocation:

Supporting Programmed Activities: 0.25 Programmed Activities per trainee per week (from existing Supoorting Programmed Activity allocation, unless explicitly stated).

    Recommended Session Allocation:

    Supporting Programmed Activities: One hour for each appraisal meeting (3 per placement).  0.25 Programmed Activities per trainee per week (if also the named trainee's Clinical Supervisor).

    Contracted:

    Measured Outputs:

    • Provision of Supervisor's report
    • Communication with Specialty Tutor/Foundation Programme Director
    • Participation in Workplace based Assessments, Annual Review of Competence Progression (ARCP) process and annual General Medical Council (GMC) Trainer survey
    • Variable requirement to attend local education, Specialty Training Committee and recruitment meetings (deputising for Speciality Tutor as appropriate)

    Comment:

    Some Educational Supervisors may have additional trainees and extra Programmed Activities may be needed. This may be managed within a Department, whilst maintaining on average 2.5 Supporting Programmed Activities per Consultant.

    Educational Supervisor (GP)

    Recommended Session Allocation:

    Supporting Programmed Activities: 

    • One hour for each appraisal meeting (3 per placement).
    • 0.25 Programmed Activities per trainee per week (if also the named trainee's Clinical Supervisor).

    Contracted: 2 sessions per week

    Measured Outputs:

    • Educational Personal Development Plan.
    • Attendance at local and school education events and experienced trainer courses (5 days per annum over 3 year cycle) Reapproved every 3 years - subject to satisfactory assessment visit.

    Comment:

    Previously known as GP Trainer. Usually also undertakes the majority of clinical supervision in a practice.

    Specialty Tutor

    (This role is ideally combined with College Tutor role)

    • Maintains an environment within the departmental multi-professional team that supports training and delivers the curriculum andrelevant assessments at Foundation, GP and Specialty levels.
    • Supports trainees and trainers.
    • Attends departmental and Trust Education Committees and will usually be required to attend Deanery Specialty Training Programme or School Board meetings.
    • Ensures systems are in place for specialty induction, quality control of training, delivery of formal education and study leave management.
    • Involved in careers guidance and trainees in difficulty.

    • Director of Medical Education
    • Training Programme Director
    • College or Faculty (if required - usually channelled through the Training Programme Director).

    Recommended Session Allocation:

    Supporting Programmed Activities:

    • 1 Programmed Activity for up to 20 trainees (excluding Foundation School)
    • 1.5 Programmed Activities for 20-40 trainees
    • 2 Programmed Activities more than 40 trainees. Larger departments may elect to share the responsibility.

    Contracted: In large Specialties, this is not from Supporting Programmed Activity time and will require a reduction in clinical Programmed Activity time.

    Measured Outputs:

    • Review of quality indicators including General Medical Council (GMC) and Deanery Surveys
    • Annual departmental report
    • Participation in: recruitment process, local education, Annual Review of Competence Progression (ARCP) and Specialty Training Committee meetings and annual General Medical Council (GMC) trainer’s survey

    Comment:

    Jointly appointed by the Trust Director of Medical Education (or Medical Director) and the Deanery (Training Programme Director). Most will also have Supervisory responsibilities. A Tutor is needed on each major clinical site, a minimum of 1 Programmed Activity per site. This role is now pivotal for educational governance purposes.

    Recruitment Support

    (in addition to other training roles)

    • Shortlisting and interviewing for Deanery.
    • Foundation - 1 round per year
    • Core training - 1-2 rounds per year
    • Specialty training - 2-3 rounds per year.

    All Training Programme Directors, most Specialty Tutors and some Educational Supervisors will spend time on recruitment each year. Every department who receives trainees should participate in the process on a pro-rata basis.

    Supporting Programmed Activities: Additional professional leave may be required e.g. 1 - 6 days per year.

    Attendance at interviews and preparation of material for interviews. 

    Comment:

    Centralised recruitment is a more efficient use of Consultant time. Recruitment to Core Medicine and Core Surgery requires the equivalent of 1 day for short listing and up to 3 days for interviewing in each recruitment round. 1-2 days/round for Specialty.1-2 days/year for Foundation.

    Specialty Association Committees and Regional Advisor Roles

    • Provides an important link between the Colleges/Specialist Society and the workplace for both postgraduate training and Continuous Professional Development for consultants.
    • Ensures consistency in approach and standards between provider organisations across the UK.
    • Provides an external view on training from a broad perspective.

    Local hospital often benefits from national recognition and knowledge gained. Colleges provide expenses and Hospitals are expected to release Consultants to attend.

    Director of Medical Education (DME)

    Maintains and develops profile of medical education within Local Education Provider, to ensure delivery of Education Contract quality control of all Postgraduate Medical Education (PGME) training programmes.

    Accountability:

    Fiscal: Trust / Deanery through the Strategic Health Authority Service Level Agreement.

    Reporting:

    • Trust (Medical Director)
    • Deanery

    Recommended Session Allocation:

    Supporting Programmed Activities: Supporting Programmed Activity contribution - subject to local job plan.

    Contracted: 1-4 Programmed Activities-dependent on size of Local Education Provider. Joint Deanery/Local Education Provider funded.

    Measured Outputs:

    • Review of quality indicators -including General Medical Council (GMC) and Deanery Surveys
    • Annual Report.
    • Participation in Deanery meetings (4-6 per year)
    • Chair of local educational meetings (3-4 per year)
    • Appraisal with Deanery and Medical Director

    Comment:

    Local individual negotiation, whether additional paid Programmed Activity reduction in clinical Programmed Activity time. Joint Deanery/Local Education Provider appointment. (Local Education Provider in Peninsula Deanery)

    Associate Director of Medical Education

    (with defined responsibility) (Associate Director of Medical Education/ College Tutor in Peninsula Strategic Health Authority)

    • Supports Director of Medical Education and Trust Educational Team with trainees in difficulty.  Provides pastoral and career support to trainees as necessary.
    • Assist in the management of the Postgraduate Centre.

    Recommended Session Allocation:

    Supporting Programmed Activities: Supporting Programmed Activity contribution (subject to local job plan)

    Contracted: 1-2 Programmed Activities, dependent on size of Local Education Provider.

    • Participation in local education meetings
    • Deputise for Director of Medical Education, as required
    • Appraisal with Director of Medical Education (and Medical Director, if desired)

    Head of School (HoS)

    Provides unifying link for Training Programme Directors within a broad Specialty. Liaises closely with the Deanery and College to ensure delivery and quality management of Curriculum delivery. Manages the Study Leave budget.

    Recommended Session Allocation:

    Supporting Programmed Activities:

    Contracted: 0.5 - 2 Programmed Activities (allocation dependent on School size). Trust reimbursed for sessions via invoice.

    • Appraisal with Deanery
    • Conducts appraisals for Training Programme Directors

    GP Dean / Head of School

    Strategic development and leadership of GP education. Ensures delivery of the Curriculum and quality management of the delivery on behalf of Deanery. Manages budgets and salaries for General Practice Specialty Trainees (GPSTRs) in community placements. Works regionally with Strategic Health Authority and Trusts and nationally within Committee of General Practice Educational Directors (COGPED), College and Department of Health.

    Foundation Programme Director (FPD)

    Manages foundation training programmes across a locality-based group of Local Education Providers on behalf of Deanery-based Foundation School.

    Accountability:

    Fiscal: Trust / Deanery through the Strategic Health Authority Learning Development Agreement

    Reporting:

    • Trust (Director of Medical Education)
    • Director of Foundation School (Deanery)

    Recommended Session Allocation:

    Supporting Programmed Activities:

    Contracted: Budget allocated to Local Education Provider for role (1 Programmed Activity / 30 Foundation trainees).

    Measured Outputs:

    • Provides annual report
    • Participation in recruitment and quality review processes
    • Appraisal with Director of Medical Education and Director of Foundation Programme

    Training Programme Director (TPD)

    Manages specialty training programmes, across a locality based group of Local Education Providers, on behalf of Deanery-based School.

    Recommended Session Allocation:

    Supporting Programmed Activities: Supporting Programmed Activity contribution (subject to local job plan).

    Contracted: Budget allocated to Local Education Provider for role (pro rata).

    Measured Outputs:

    • Participation in recruitment process
    • Participation in Annual Review of Competence Progresion (ARCP) and Specialty Training Committee meetings

    “Patch” Associate Postgraduate Deans lead provision of postgraduate medical education in each programme. “Portfolio” Associate Postgraduate Deans lead on specific work streams including: appraisal, assessment, Quality Assurance, foundation training, post Certificate of Completion of Training (CCT) education, practice nursing, recruitment, remediation, induction and refresher scheme (Returners).
