If you would like to provide photos for use by Severn Postgraduate Medical Education or any of the Schools, in any media (eg websites, training materials etc), we need to ensure we have permission (Copyright) to use them.
We have produced the following forms:
Model Release Form - If you take a photo that includes identifiable people, they need to give permission for us to use the photos in any publicity media. They will need to sign this form.
Photographic Release Form - If you take a photo with no identifiable people, scenery, etc you will need to give permission for us to use the photos in any publicity media. You will need to sign this form.
Important: All signed forms must be returned to either your School Manager, or the Website Administrator.
We will be unable to use photos without these signed forms.
